COVID-19 Screening, Consent Form, NCHSAA Pre-participation and FamilyID will need to be completed by each student athlete/parent before participation in athletics.

(Health History Form) (We only need the first 2 pages completed, unless your physical has expired) If your physical was after 3/1/2019 you are good for this years athletic season.

If your child has other special needs to be assessed annually, or know that they were
exposed to or diagnosed with Covid 19, please reach out to their physician; as they may need to provide their annual note of visit for this (this is common with concerns such as in cardiology).

At this time, we are assessing which athletes may need to obtain a physical due to not falling into this timeframe.  If your child will need one due to having a physical prior to March 1st, 2019, locations they can go to include; Southeastern Sports Medicine Monday-Friday (it is recommended that you call 274-4555 prior to just showing up to make sure a provider is there), most Urgent Cares, or your Family Doctor.

* 6th Graders: If you are interested in managing a sport team, you must also have a current sports physical on file with the Athletic Department and Register on Family ID
If you need a physical form, please select the NCHSAA Physical Form tab located on the left and take the forms to your physicians office.
